Title: Canada Student Loan Forgiveness for Family Doctors and Nurses Application Form

Purpose: A form for family doctors, residents in family medicine, nurse practitioners, or nurses to apply for Canada Student Loan forgiveness if they have worked in a designated rural or remote community anywhere in Canada, meet other eligibility requirements, and have Canada Student Loans to repay.

Title: Repayment Assistance Application

Purpose: The Repayment Assistance Plan (RAP) application form is used to apply for the RAP: a benefit that is available for student loan borrowers who are having difficulty repaying their Canada Student Loans. You must apply for RAP as enrolment is not automatic. If you have student loans from Alberta, British Columbia, Ontario, New Brunswick, Newfoundland and Labrador, Nova Scotia or Saskatchewan, this single application will cover both your Federal and Provincial loans under the applicable Federal and Provincial RAP programs and Interest Relief for some provincial loans. If you also have a Canada Student Loan with a Financial Institution, your application will also apply to that loan.
How To Apply: If your Canada or Integrated Student loans are held by the National Student Loans Service Centre (NSLSC), you can apply for RAP online through your NSLSC account (you can register for an account first if you do not have one). You can also download an application below, fill it out and mail or fax it to the NSLSC.
If you're applying for Repayment Assistance for a Canada Student Loan negotiated before August 1, 2000 that is held by a financial institution (and you don't have a Canada Student Loan through the NSLSC), you must submit your application and supporting documentation to your financial institution directly. If you have a provincial or territorial student loan from a jurisdiction that is not listed, ask your Provincial or Territorial Student Assistance Office about any programs they have for students having trouble repaying their loans.
Under RAP you are approved for a six-month period. Borrowers must re-apply every six months in order to maintain their eligibility.

Title: Repayment Assistance Plan for Borrowers with a Permanent Disability (RAP-PD) Expenses Form

Purpose: Use this form to provide information about your uninsured, permanent-disability-related expenses when you apply for the Repayment Assistance Plan for Borrowers with a Permanent Disability (RAP-PD). Your disability-related expenses may impact your eligibility for the RAP-PD or the amount of your monthly loan payments.
How To Apply: If your Canada or Integrated Student loans are held by the National Student Loans Service Centre (NSLSC), you can apply for the Repayment Assistance Plan online through your NSLSC account (you can register for an account first if you do not have one). The online application also allows you to print the Repayment Assistance Plan for Borrowers with a Permanent Disability (RAP-PD) Expenses Form, fill it out and upload it with your expenses.
You can also download the Repayment Assistance Plan application and the RAP-PD Expenses Form below, fill them out, and mail or fax them to the NSLSC.
If you're applying for Repayment Assistance for a Canada Student Loan negotiated before August 1, 2000 that is held by a financial institution (and you don't have a Canada Student Loan through the NSLSC), you must submit your application and supporting documentation to your financial institution directly.
How to use this form: Print, complete, sign and send this form to the NSLSC for assessment. If your expenses are deemed eligible, they will be applied to your current, approved RAP-PD period. Remember, you must re-apply for the RAP-PD every six months.

Note: To be eligible, expenses must be uninsured, directly related to your permanent disability, and paid for during the month of your RAP-PD application or during the months for which you were required to provide proof of income.
Eligible expenses will be deducted from your gross family income amount. Additional expenses may be considered based on the extraordinary expenses criteria under the RAP reconsideration process. If you have included additional extraordinary expenses on your form, read and sign section 5 (page 2) so they may be considered.

Title: Confirmation of Enrolment (Schedule 2)

Purpose: A form that borrowers and post-secondary institutions complete as proof that a borrower is enrolled in school. This document keeps your student loan in interest-free status and ensures that you do not begin repayment of your loan while you are still in full-time studies, or to remain in interest only status if you are studying part - time. For borrowers from Ontario, please contact your provincial or territorial student assistance office.
Download and print the file. Ensure you complete and sign the consent and certification section and have an authorized officer of your school complete and sign the confirmation of enrolment section.

Title: Bank Account Change request form (while in study)

Purpose: Borrowers who are in-study and who signed a Master Student Financial Assistance Agreement (MSFAA) can use this form to update their banking information. Note: non-MSFAA borrowers can update their banking information before each loan and/or grant disbursement on their Certificate of Eligibility and Student Loan Agreement.

Title: Pre-Authorized Payment form (while in repayment)

Purpose: All borrowers who are repaying their loans can use this form to sign up for pre-authorized payments or to update their banking information. Note: the NSLSC can only keep one bank account on file for all your loans, so you only need to complete this form once even if you have more than one loan.

Title: One-Time Payment form

Purpose: To enable borrowers in repayment to make lump sum or extra payments on their student loans by requesting a one-time payment withdraw from their bank account.
Print this form and complete the required fields. Sign, date, and fax or mail it to the address indicated on the top of the form. Please remember to attach/fax a copy of your void cheque with the form.

Title: Change Your Payment Due Date

Purpose: Use this form to change the day of the month your payment is due. Payments must be due on the same day of each month (for example the 10th day of each month), however the payment date can be different for each loan.
To make payments more often than monthly (bi-weekly, weekly etc.), you must cancel your pre-authorized payment and then set up the National Student Loans Service Centre as a bill payment using internet banking. Your loan number (without the dash) is your account number. Please ensure that your total payment amounts/month equal or exceed your regular monthly payment amount to ensure that your loan does not go into arrears.
Print this form and complete the required fields. Sign, date, and fax or mail it to the address indicated on the top of the form. Please remember to attach/fax a copy of your void cheque with the form.

Ontario students receiving Canada-Ontario Integrated Student Loans must complete and submit two (2) Power of Attorney forms: an HRSDC Power of Attorney form and an Ontario Power of Attorney form.

Note: This form is for you if you have signed a Master Student Financial Assistance Agreement (MSFAA) or you are in the process of signing and submitting your MSFAA and wish to designate a Power of Attorney. A separate POA form issued by a solicitor is required if you have not signed an MSFAA.
